- On the patient's Compliance tab, choose Certification of Terminal Illness → Start →.
- Jubal reads every record and takes the terminal diagnosis from hospice sources only. If no hospice source states it, Jubal proposes the best-supported one and asks you to confirm or correct it.
- The certification is one page: identifiers, the certification sentence, the diagnosis, a blank space for the physician's narrative, the attestation, and a blank signature block.
- Jubal does not write the narrative. Medicare requires the certifying physician to compose it and attest to that above the signature (42 CFR 418.22(b)(3)). The physician writes it on the printout or types it into the document.
- Beside it Jubal builds Clinical findings for the certifying physician: what the records document, each finding with its record and page. It states no prognosis and is not signed.
Redo → rebuilds both from the current records.
