The election statement addendum

Since October 1, 2026 the addendum must be furnished to every Medicare patient within five days of the election, filed with the election statement, and updated within three days when a plan-of-care change affects a determination. It is a condition of payment.

The addendum comes from the finalized relatedness worksheet: it lists, in plain language, what the hospice has determined unrelated and will not cover.

1. Finalize the relatedness worksheet first.
2. On the Compliance tab choose Election statement addendum → Build →, or open the worksheet and choose Build the addendum.
3. Rows marked for physician review stay off until the physician decides.
4. When a later worksheet changes a determination, the row shows Update → and the three-day clock starts.

The patient's signature acknowledges receipt only. If the patient declines to sign, note it on the form.
